Help patients take control of their own health
When a patient called confused about her Cabozanix 20mg (Cabozantinib) multiple medications, Christina Wilson-Smith was there to help her. Our certified pharmacist, who has been with Express Scripts ® Pharmacy since 2007, instructed the patient to find all of her prescription bottles and a black permanent marker so she could write on the bottle cap or label. For 30 minutes, they both reviewed each medication, one by one.
It turned out that the patient was confusing the doses of the cholesterol medication and the blood pressure medication. She also didn't have any of her heart medications on hand. She assumed it was the same medication as another one she was taking because her names sounded similar to her.

Support a patient after a new diagnosis
Mike Martinelli contacted a patient after seeing that she had recently been diagnosed with type 2 diabetes and was prescribed a medication to regulate her blood sugar. The patient felt overwhelmed and she explained that when she picked up testing supplies at her local retail pharmacy, the pharmacist there did not have time to show her how to test her blood sugar.
Martinelli scheduled a call with the patient for the next day, when she would not have any distractions. The next day when they talked, he walked her through the entire testing process, step by step.
